How to use these forms

  1. Print the form on A4 paper — or collect a printed copy from any of our offices.
  2. Fill in every field in ink, in English or in Malayalam.
  3. Sign wherever the form asks for a signature. For a joint account, every account holder must sign.
  4. Submit it in person at any branch, together with the supporting documents listed on the form itself — usually proof of identity, proof of address and photographs.
  5. Our staff will check the form with you before it is accepted, so anything missing can be completed on the spot.

Forms are available at the branch too

Every form shown here can also be collected free of charge at the Head Office in Thondiyil and at our Nedumpoyil and Poolakkutty branches. If you do not have a printer, or are not sure which form you need, simply ask at the counter — our staff will help you fill it in.

Banking hours: Monday – Friday 9.30 am – 5.30 pm · Saturday 9.30 am – 2.00 pm · Sunday closed.
